Our goal is to help you find and understand the causes of your health challenges and create a customized treatment plan for you. During this visit, Elaine Hardy will be performing a comprehensive medical history and physical exam in order to fully understand your condition.

Your treatment plan will likely consist of significant changes to your normal diet and supplement regimen. Please be sure that you are motivated to make these changes before you meet with us. We prescribe dietary changes, and nutritional supplements such as vitamins, minerals, enzymes, amino acids, essential fatty acids and herbs. We will occasionally prescribe medications, but only when necessary.

If warranted, we may order in-depth laboratory testing of blood, saliva, hair, urine, breath, and/or stool. This testing is more comprehensive than traditional laboratory testing and may utilize specialty laboratories.

Once you have scheduled your first appointment, new patient forms will be emailed to you from our office.  Please take the time to fill out the forms completely prior to coming to the office to avoid delaying your appointment.  If you do not receive your new patient forms after making an appointment, please be sure to contact our office and let us know.

To make the most of your first appointment, please bring along copies of any recent test results. In addition, please bring a complete list of the nutritional supplements and medications you take, with exact ingredients (If in doubt, bring the supplements themselves). Our aim is to make your visit as comprehensive and informative as possible.

The initial consultation usually takes between 1 and 1-1/2 hours depending on the complexity of your situation so please budget enough time.

Insurance

We do not accept insurance of any kind. We will provide you with a detailed receipt for you to submit to your insurance company. We do not submit to insurance companies.

Fees

Your INITIAL visit will be billed at a flat rate of $292. Charges for subsequent visits are based on an hourly rate of $292/hr. (new rate as of 1/1/2023). Charges are billed in 15 minute time increments. Patients will be billed for extensive emails and for telemedicine appointments. We accept all forms of payment including cash, checks, and all major credit cards.

Specialty Lab Testing

Specialty testing varies depending on what test we order, and payment is to be arranged directly with the lab performing the testing. We offer no guarantee that your insurance company will cover ordered laboratory testing.
